Description

JOB DESCRIPTION:

  • Clinical Expertise: Proficient clinician with broad patient care experience and deep understanding of specific patient populations. Utilizes knowledge, theory, research, and experience to adapt to clinical changes.
  • Synergy Model for Care: Applies the Synergy Model to personalize patient care, serving as a resource to colleagues and advocate for patients and families.
  • Effective Communication: Consistently communicates respectfully with patients, families, and healthcare team members, delivering safe and effective care independently.
  • Assessment and Documentation: Demonstrates advanced assessment skills, recognizes nursing care impact on patient outcomes, and documents nursing process accurately.
  • Patient-Centered Care: Plans and implements patient care independently, incorporating patient and family input and goals, and evaluates responses to care.
  • Leadership and Collaboration: Functions in a developing leadership role, delegates appropriately, and collaborates effectively with interdisciplinary teams and care partners.
  • Professional Development: Accepts accountability for professional growth, participates in discipline-specific issue identification and resolution, and articulates application of Synergy model in clinical practice.
  • Data Analysis and Improvement: Collects, reports, and analyzes data, identifies areas for improvement, and revises patient care documentation as needed.

MINIMUM QUALIFICATIONS:

  • Education and Experience: Graduate of accredited nursing school with recent nursing experience. Valid, active unencumbered nursing license or temporary permit approved by Georgia Licensing Board.
  • Certifications: BLS certification required, ACLS may be required in designated departments. LPNs transitioning to RN roles within the EHC system eligible for Nurse Clinician title.

PHYSICAL REQUIREMENTS:

  • Medium-Heavy lifting, occasional to constant, depending on workday tasks. Physical demands may vary based on assigned work area.

ENVIRONMENTAL FACTORS:

  • Environmental exposures vary by work area and tasks, including blood-borne pathogen exposure, chemicals, communicable diseases, and more. Use of personal protective equipment may be required.
