The MPI RN Educator PCF assesses, plans, implements, and evaluates the patient centered diabetes education program at the Diabetes Center. Plans and conducts continuing education programs, participates in the development of education curricula and materials. Participates in case management, and patient care planning and goal setting. Provide diabetes education services and general diabetes self-management education.

Responsibilities

A day in the life of an MPI RN Educator PCF at Hackensack Meridian Health includes:

  • Works directly with clinic and center staff to develop and implement new and existing clinical/education programs that service patients, professionals and the general public.
  • Participates in outcome research of education and clinical programs offered at the Center.
  • Participates in the development of education curricula and materials by writing original material and/or reviewing the work of others.
  • Participates in quality control/quality improvement activities within one¿s scope of practice that promotes quality service.
  • Applies nursing process and education of patients and families, in a variety of settings, encouraging the team approach to diabetes management, according to standards.
  • Assess and evaluate patient's management and educational needs, self-care behaviors, obstacles to successful implementation of therapy in a variety of settings.
  • Develop individualized education management plans including appropriate follow-up.
  • Provide formal training on diabetes management and risk factor reduction.
  • Provide management consultation in person and over the phone to patients/care providers/community workers.
  • Coordinate consultations and referrals and facilitate communication between patient and providers as needed.
  • Measure and document, through written tests and/or verbal discussion and demonstration, patients' understanding of information taught.
  • Documents and processes medical records, billing and statistical records according to Policy and Procedures.
  • Maintains professional registration and certification & stays up to date on current thought and research in diabetes, diabetes management and behavior change strategies by attending conferences, reading relevant literature, and using Internet resources as available.
  • Contributes to Center goals for outreach and increased visibility by maintaining active involvement in professional organizations (such as contributing to newsletters and articles, delivering lectures, and contributing to development and implementation of workshops.
  • Other duties and/or projects as assigned.
  • Adheres to HMH Organizational competencies and standards of behavior.

Qualifications

Education, Knowledge, Skills and Abilities Required:

  • BSN.
  • 1 or more years of experience in diabetes care and education/counseling of patients with chronic disease
  • 3 or more years of experience as a nurse, preferably one year in community health or ambulatory care setting.
  • Excellent written and verbal communication skills.
  • Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.

Education, Knowledge, Skills and Abilities Preferred:

  • Master's Degree.

Licenses and Certifications Required:

  • Valid NJ Registered Nurse licensure.
  • Certified Diabetes Educator (CDE) or Board Certified.
  • Advanced Diabetes Management (BC-ADM).
