Required:
* Associate's degree or equivalent from 2-year college or technical school AND 2-4 years related experience & /or training; OR equivalent combination of education & experience
* OHA certification as a Traditional Health Worker or obtain within 6 months of employment
* Valid driver's license, acceptable driving record & auto liability coverage or access to an insured vehicle
Summary:
This position is responsible for the planning, organization & execution of Health Equity outreach activities, including building & maintaining positive working relationships & participating community partners, as well as fulfilling regulatory requirements. Will operate as the facilitator of the local Community Information Exchange (CIE) website, ensuring that offered resources are properly claimed, updated & responded by the corresponding community partner. Will also serve as the primary coordinator for the Community Advisory Council (CAC), organizing & corresponding with internal & external partners to facilitate meetings & activities.
Essential Duties:
1. Works alongside the community & partner organizations in delivering culturally responsive resources to members & community by planning, organizing & attending outreach activities.
2. Serves as the CAC Coordinator & the Systems of Care (SOC) Coordinator, which includes scheduling meetings, preparing agendas, minutes & meeting packets. Facilitates communication between the CAC, SOC & the leadership team of the organization, ensuring the voice of the member is heard.
3. Conducts outreach to community partners & provides them with technical assistance to increase the number of claimed & utilized programs on the CIE.
Duties Include:
* Collaborates with internal teams to coordinate outreach efforts & ensure alignment with organizational goals
* Maintains accurate records of outreach activities, including participant info & outcomes
* Assists with administrative tasks related to outreach activities
* Plans & organizes regularly scheduled & special outreach events, ensuring proper assignment of resources, identifying activity effectiveness & potential areas of improvement
* Assists with development of the CIE & community & member outreach to increase use of the CIE
* Schedules, coordinates & records CAC & SOC activities
* Coordinates the administration of the Non-Emergency Medical Transport program
* Assists with the development of policies & procedures, workflows, flyers & training materials
* Analyzes & reports on Key Performance Indicators
* Facilitates program & project team implementation of health equity, social determinates of health & member experience programs & initiatives
* Completes special projects or events which may require researching, reviewing & analyzing data, identifying best practices, recommending solutions & writing reports, action plans & program or project plans
* Attends state & community meetings & events as organization representative
* Maintains confidentiality & complies with HIPAA rules & regulations
