Harborview Medical Center – Social Work has an outstanding opportunity for a SOCIAL WORKER to join the Madison Clinic.

Candidates for this position must be bilingual and Spanish-speaking.

WORK SCHEDULE

DEPARTMENT DESCRIPTION
The Madison Clinic is an interdisciplinary, outpatient primary care clinic located at Harborview Medical Center that serves individuals living with HIV and/or at high-risk of acquiring HIV. The Madison Clinic case management program receives federal grant funding to provide HIV case management services to the patients receiving medical care at Madison Clinic. Many of the patients served at Madison Clinic have complex psychosocial needs that make navigating the health care system challenging. The primary goal of case management services is to reduce barriers to medical care and to increase access to medication for HIV-positive patients. Some of examples of the barriers often seen are lack of adequate insurance, substance use, stigma, complicated immigration statuses, mental health issues, and homelessness.

PRIMARY JOB RESPONSIBILITIES:
• Provide HIV/AIDS intake and case management to adult patients served in Madison Clinic.
• Conduct comprehensive assessments of psychosocial, financial, medical support and environmental needs
• Develop individualized case management service plans to assist the hospitalized and ambulatory care patient to obtain needed medical, social, financial, legal and home care services, housing, psychiatric and drug treatment and emotional support

REQUIRED POSITION QUALIFICATIONS:
A Master of Social Work Degree from a program accredited by the Council on Social Work Education
AND
One year of full-time social work experience in a health care setting or equivalent. Experience can include a practicum placement as part of a social work training program and/or employment experience.

LEGAL REQUIREMENTS:
Within 90 days of hire must hold one of the following credentials issued by the state of Washington:
• Agency Affiliated Counselor
• Licensed Social Work Associate -Advanced
• Licensed Social Work Associate - Independent Clinical
• Licensed Advance Social Worker
• Licensed Independent Clinical Social Worker
