Details:Pharmacists work primarily first shift with some second shift
requirements. Start times can be as early as 0630 and end times as late
as 2330. Shifts are typically eight hours and our team staffs every
third weekend. Pharmacists staff both central and decentral. This
position is located at Oconomowoc Memorial Hospital and the pharmacist
in this role will also be trained at Waukesha Memorial Hospital.
Pharmacist at times may be required to enter ante/buffer rooms and hood
spaces, and as a requirement from regulatory bodies, must be jewelry and
makeup free. What You Will Do: Promotespositive patient outcomes for
patients and families of ProHealth Care utilizing pharmaceutical
care.The pharmacist incorporates and demonstrates clinical skills and
customer service strategies in order to achieve the organization\'s
goals. Responsibilities: Enter or verify physician medication related
orders. Verify/ approve the work of Pharmacy Technicians Follows
guidelines of the P&T committee for therapeutic interchange, and
formulary management. Monitors drug therapy for appropriateness and
promote/ evaluate rational drug therapy that improves pharmaceutical
care. Performs pharmacokinetic consultation. Improves patient care by
shortening length of stay, preventing ADR and controlling pharmaceutical
costs Will work with physicians, specialists and other clinicians to
implement appropriate guidelines and protocols for drug use based on
evidence based medicine, national guidelines, and literature based
protocols. May act as a preceptor to pharmacy students or pharmacy
residents. Provides mentoring for pharmacy students on rotation
consistent with college requirements. Provides drug information to
nurses, physicians, and other pharmacists to ensure current information
