Responsibilities Job Description

The LPN performs delegated nursing and medical functions of data collection and delivery of care to an assigned group of patients, or individualized patient. The LPN works under the direction of a Registered Nurse, or physician/authorized practitioner and in collaboration with other members of the healthcare team. The LPN is responsible for adhering to all policies, procedures, and nursing practice guidelines.

  • care for simple and complex wounds and skin care injuries

  • provide care of service adjusting approaches to reflect developmental level and cultural differences of population served

  • Ability to use Hoyer lift, gait belt or slide board to transfer patients from their mobility device to exam chair or stretcher

  • Knowledge of wound healing, knowledge of wound etiology, and knowledge of topical wound therapies

  • Perform daily cares such as pericare when appropriate

  • Knowledge of current pressure ulcer practice guidelines, pressure ulcer etiology and treatment modalities

  • Knowledge of current best practices in wound care

  • Knowledge of cultural and age related issues for wound care patients

Qualifications

Required

· Vocational/Technical Training or Associates Degree

· Current MN LPN License

Preferred

· One to three wounds careyears

· Previous ambulatory Care experience.
