Description

Position Summary

Responsible for performing aspects of the Patient Flow Management process ensuring that transfers, patient placement and related activities occur in a timely, safe and accurate fashion. This includes providing clinical assessment for patients being placed through the Center for Patient Flow Management.

Essential Functions


Coordination of incoming patient transfer requests with receiving facility, physicians, and necessary transport resources
Demonstrates technical skills in care delivery that result in safe practice and positive care outcomes
Demonstrates clear communication and effective collaboration to optimize patient care and clinical operations
Displays leadership that influences optimal clinical and operational outcomes
Delivering exceptional customer service while facilitating communication across multiple disciplines
Possess and obtain working knowledge of external and inter-facility protocols and standards of practice
Work with all Center for Patient Flow Management (CPFM) staff in a team orientated, collaborative approach
Responsible for promoting a Best Place to Work Culture and positive and professional work environment
Radio communication and coordination with multiple agencies, including Air Medical programs and UH Critical Care Transport
Dispatch of UH Critical Care Transport ambulances
Other duties as assigned
