Position Overview: As a Care Manager, you will collaborate closely with adults aged 18 and older, along with their families and caregivers, to conduct comprehensive biopsychosocial assessments and develop individualized care plans. Your responsibilities will encompass implementing and monitoring these plans, ensuring the provision of home and community-based long-term supports and services. Essential duties include providing ongoing support and information to clients and their families, maintaining robust relationships within the health and human service community, and advocating for clients to access formal and informal care and services. You will also maintain accurate and HIPAA-compliant participant files and program records, conducting in-home assessments throughout Berrien, Cass, and Van Buren Counties.
About the Program: The MI Choice Waiver Program, formerly known as HCBS/ED, offers Medicaid-covered services akin to those in nursing homes but allows eligible adults to remain in their own home or community setting. Established by Michigan Medicaid in 1992 and available statewide since 1998, it provides basic Medicaid services along with support coordination and various additional services, including adult day health care, counseling, nursing services, and more, catering to individuals' specific needs and promoting independence
Qualifications:
- Bachelors or Masters degree or Masters Degree in Social Work or Nursing
- Current MI Licensure as an RN, LLBSW, LBSW, LMSW, or LLMSW
- Experience in working with older adults and/or persons with disabilities preferred.
- Proficiency in standard office applications and computer literacy required (Microsoft 365 products).
- General understanding of long-term care, person-centered thinking, and community resources preferred.
- Ability to handle and prioritize multiple tasks is essential.

- Full-time position (37.5 hours/week), no weekends or on-call duties.
- 50/50 hybrid schedule after training period
- Flexible scheduling between the hours of 6am and 7pm Monday- Friday
- $200 continuing education/ per year

Key Responsibilities:
- Provide ongoing support and information to clients, families, and caregivers, maintaining strong relationships within the health and human service community.

- Develop, implement, and monitor person-centered care plans following established guidelines and standards.

- Advocate for clients and facilitate access to formal and informal care and services through external sources.
- Maintain accurate and HIPAA-compliant participant files and program records, including documentation of in-home assessments conducted throughout Berrien, Cass, and Van Buren Counties.
