University of Minnesota Physicians is seeking a Certified Nurse Practitioner or a Certified Physician Assistant who will deliver care for patients on the University of Minnesota Heart Care team at the M Health Fairview Heart Care Clinics in Maplewood and Woodbury and St. John's Hospital in Maplewood. The position will provide patients with expert patient care and coordination of services. They will assist the practice to improve patient satisfaction and enhance the quality of care provided to patients through increased access and coordinated services.
In This Role, You Will:
Join a multidisciplinary team of 20 Cardiologists and 11 Advanced Practice Providers
The PA or NP provides and coordinates care for adults with cardiac diseases throughout the course of evaluation and treatment.
Participates in inpatient and outpatient management of general cardiology patientswith heart failure and othercardiac diseases of various etiologies.
Providespatient and family education as it relates cardiac diseases and general cardiology.
Performs pre and post procedure care and clinic follow up visits.
This individual will show commitment to exemplary patient care.
Manages patient care through appropriate use of available resources.
Demonstrates ability to deliver care or service adjusting approaches to reflect developmental level of population served.
Use a fully integrated Electronic Medical Record (EPIC)

Position is a hybrid of inpatient and outpatient care
We're Looking For a PA or NP Who Is:
A graduate of an accredited Physician Assistant or formal Adult Nurse Practitioner training program.
Eligible or is certified through NCCPA, ANCC or AANP.
Licensed or eligible to acquire a license and prescriptive authority through the Minnesota Board of Medical Practice (PAs) or Minnesota APRN license or eligible to acquire a license and prescriptive authority through Minnesota Board of Nursing (NP's).
Able to meet threshold criteria/qualificationsfor credentialing and privileges.
