Primary Function/General Purpose of Position

The Social Worker provides direct patient care under the guidance of a designated healthcare professional in accordance with federal, state, and local regulations, and within policies, procedures, and guidelines of Bon Secours Mercy Health (BSMH). May provide clinical and administrative supervision.

Essential Job Functions

  • Provides individual and family psychotherapy

  • Group facilitation, participates in family and treatment team meetings

  • Discharge planning

  • Community resource knowledge and collaboration

  • Psychosocial assessments

  • Knowledge of involuntary processes; Knowledge of guardianships, advanced directives, and POAs

  • Substance use disorder (SUD) screenings

  • SUD discharge planning

  • Preadmission Screening and Resident Review (PASRR) completion

  • Post-discharge calls

  • Patient health questionnaire (PHQ-9), Columbia-Suicide Rating Scale (C-SSRS), and social determinants of health (SDOH) screenings

  • Crisis assessment and referral services

  • Successfully participates to complete BSMH and departmental specific training as required

Licensing/Certification

Licensed Social Work (LSW) (preferred, requried within 6 months of hire)

Licensed Masters Social Worker (LMSW) (preferred)

Education

Bachelors, Social Work (required)

Masters, Social Work or Social Administration (preferred)

Work Experience

1 year in social or human services setting. (required)

Patient Population

Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit.

Demonstrates knowledge of the principles of growth and development of the life span and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his or her age, specific needs and to provide the care needed as described in departmental policies and procedures.​
