  • An RN, PT, PTA, OT, LCSW or Certified Care Manager (CMC, or CCM) with significant experience in a medical setting.
  • Passionate about care especially for older, frail, and vulnerable adults.
  • Well established in the local eldercare community and enjoy networking, outreach, and marketing to fellow healthcare professionals like case managers, discharge planners, social workers, and administrators.
  • Very familiar with the home care industry, have worked in home care, or have referred cases out to home care organizations.
  • You are also a learner always looking to improve and to grow your skills and will embrace coaching and training.
  • Emotionally strong and can work through difficult family dynamics and challenging dementia cases.
  • Willing to meet client needs while in their homes conducting assessments, Caregiver introductions and training, and quality assurance visits.
