Required Skills:
Qualifications:
• Four year degree in human services or related field with one year for direct care to individuals with disabilities or individuals of 65 years of age or older
• OR Two year degree in human services or related field with two years of direct care to individuals with disabilities or individuals of 65 years of age or older.
• Demonstrated interpersonal skills that quickly build rapport with customers, county social workers, and administrative staff.
• Excellent verbal, written, and phone communication skills.
• Ability to manage multiple priorities and take initiative with work objectives.
• Strong analytical and financial acuity skills.
• Ability to work independently; organize work consistently and meet deadlines.
• Demonstrated skills in problem-solving with an optimistic and compassionate nature.
• Ability to collaborate with co-workers across departments and teams.
• Thorough understanding of disability services and funding sources available in Minnesota highly preferred.
• Case management experience related to CDCS and CSG programs preferred.
• Travel in the greater metro area when necessary is required.
• Candidate must complete and pass a background study, pre-employment drug screen; have a valid driver’s license free of violation; proof of motor vehicle insurance and reliable transportation for work purposes.

Position: FMS Service Coordinator
Department: Self-Directed Services
Location: Hybrid - Lifeworks in Richfield
Hours: (FT) exempt position, Monday-Friday, 8:00am-4:30pm
About Us:
Lifeworks Services Inc. is a nonprofit organization that serves our community and people with disabilities as we live and work together. We have day program locations in Brooklyn Park, Apple Valley, and Mankato; community employment teams throughout the Twin Cities Metro area and Mankato; and serve over 2,100 families through Self-Directed Services.
Position Overview:
Lifeworks Self-Directed Services is seeking a full-time exempt Service Coordinator to provide case management for Lifeworks Financial Management Services clients and families. As an approved Financial Management Service through the State of Minnesota, Lifeworks helps individuals enrolled in the Consumer Directed Community Supports (CDCS) and Consumer Support Grant (CSG) programs. This is a great position for a case manager seeking to empower people to use services in their own communities. The position works as part of a greater Self-Directed Services team and also has a lot of autonomy when managing their caseload. A successful candidate will enjoy meeting new people; have excellent interpersonal skills; deliver detailed work, stay organized and meet deadlines; and have a strong desire to successfully deliver services within the required guidelines of state statutes. Lifeworks is also an experienced PCA Choice provider agency and a licensed provider of Individualized Home Support, Night Supervision, Respite, and Homemaker services.

Responsibilities:
• Manage the individual’s fiscal budgets using technical tools, spreadsheets, and utilizing mathematical abilities.
• Meet individuals and their support teams to complete the intake process and review the individual’s support plan.
• Support individuals with disabilities and their families to lead self-directed lives by utilizing their funding to meet their needs and life priorities.
• Manage caseload of clients with professional customer service and mathematical skills.
• Communicate verbally, in writing and in person with the individuals we support, their support teams, county case manager, and referral sources.
• Educate individuals we support and their families to optimize their self-directed services
• Maintain knowledge and understanding of DHS regulations, Consumer Directed Community Supports (CDCS) and Consumer Support Grant (CSG) programs.

• Ability to be comfortable in a variety of environments and with a variety of people.
• Strong attention to detail and deadline oriented.
• Excellent interpersonal customer service skills.
• Strong verbal and written communication skills.
• Ability to deliver training and empower others to follow procedures, ensuring success with self-directed services.
• Proficient in computer technologies including mobile devices and software applications, including MS Office Suite with intermediate skills in Word, Excel and Outlook.
