Leech Lake Band of Ojibwe RN Diabetes Educator in Cass Lake, Minnesota

To provide quality diabetes related patient education and nursing services. Duties/Responsibilities: Functions within the scope and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rule in state of practice and licensure. Demonstrates competency and use of the nursing process. Function within policies and procedures of Indian Health Service (IHS), Tribal, and granting agency; will utilize the most current American Diabetes Association (ADA) Standards of Care as a guide for diabetes related management and education. Will utilize educational methods and principles to provide diabetes-related education, including: staged diabetes management, team approach, and others as assigned. Will provide diabetes-related education to meet the needs of patients, their families, communities and health professionals in multiple settings; e.g. Tribal clinics, community centers, schools, homes, and other sites as needed. Assist the Diabetes Center in multiple aspects, including: patient screening, lab draws, immunizations, foot exams, foot and nail care, wound care, blood glucose monitoring, education related to self-management of diabetes, education related to health promotion, case management and other aspects as needed. Will perform appropriate, accurate and complete medical record documentation. Will maintain appropriate program records in compliance with IHS, Tribal, and granting agency policies. Will assist in maintaining the most current and evidence-based educational material to be provided to clients and for use in training healthcare professionals and paraprofessionals. Collaborates with health professionals to plan, implement and evaluate care. Collaborates with other community health resources; e.g. IHS, LLBO programs and community agencies. Assist with program planning, including: development, implementation and evaluation. Assist with grant requirements, including: chart reviews, audit, data collection, reporting and as assigned. Assist in quality improvement methods. Attend and/or conduct all mandatory meetings and trainings as required Ability to travel in and out of state, possibly on a regular basis. Maintain confidentiality and professional ethics at all times. Understanding and sensitivity to Native American culture Adhere to policies governing staff conduct All other duties as assigned.
