This position is responsible for providing ongoing child protective services to children and their families. The ongoing child protection team receives cases after a family assessment or investigation is completed following reports of child maltreatment. This position is responsible for assessing and evaluating the safety, permanency and well-being needs of children. This includes evaluating and supporting family functioning by identifying family strengths and needs, developing safety plans for children/families, and engaging families in addressing the child protection concerns. As a Social Worker/Case Manager, you will: Assess the psychosocial, behavioral, medical and safety needs of clients and families to coordinate appropriate services as needed through ongoing communication with clients, families and providers. Create and implement detailed service plans to identify and manage goals, services and resources specific to each client, while maintaining paper, electronic files and database filing systems to meet federal, state and local mandates and timelines. Prepares extensive and comprehensive social histories; home studies; case plans; case records; court reports, petitions and testimony; appear personally in court to represent MNPrairie. Collaborate with state, federal and local entities; Collaborate with other service providers including schools, mental health professionals, court personnel, attorneys, home care providers and managed care organizations. Respond to and address crisis situations specific to safety of children including imminent danger to self or others is apparent or possible. Organize, provide and participate in ongoing training to increase child welfare practice, community knowledge, and social work skills. Mentor new staff and interns. Performs related work as assigned.
