Guthrie Case Manager - Home Health PA - Full Time, Days in PA, United States

Guthrie Home Health Towanda is searching for a disciplined & compassionate Registered Nurse Case Manager in the Home Health setting in the Wyalusing area in Pennsylvania. Our amazing team of Case Managers are critical thinkers that help provide a high standard of care to patients in their homes.

The hours are typically 8:00 AM - 4:30 PM but the position offers flexibility with accountability. The schedule is Monday - Friday with on-call and weekend rotation (typically 1 weekend a month).

Position Summary:

Responsible for the case management and provision of quality home nursing services to patients within the agency’s philosophy, policy, and Standards of Community Health Nursing Practice.

Education, License & Cert:

  • Graduation from an approved School of Nursing.

  • Bachelor’s Degree in Nursing preferred but not preferred

Experience:

  • One‐year previous nursing experience with Medical/Surgical Nursing required.

  • Prior Public Health/Community Health experience preferred.

Essential Functions:

  • Plans, implements, manages, and evaluates the provision of both professional and ancillary home health services (direct care and case management) to ensure that all patient’s needs are met and quality care provided in accordance with Federal, State, and program guidelines.

  • Is responsible for ensuring that he or she can meet the yearly competency requirements. Attends staff meetings and educational opportunities.

  • Must provide nursing care in the patient’s environment.

  • Manages the activities of the multi‐disciplinary team providing care to patient.

  • Demonstrates effective time management, flexibility and priority setting in patient care.

  • Must have excellent verbal and nonverbal communication skills and facilitate communication between team members and patients.

  • Independent judgement and excellent planning and problem‐solving skills are required.

  • Is responsible for LPN supervision.

  • Must have reliable transportation.

  • Must be able to accept on call assignments.

  • Travel for this position is always required.
